5. Matrix response

This question type allows an individual to select one or more pre-defined responses (column headings) for multiple situations (row questions). For example, it can be used to have responses to select a range (1-10, unlikely to likely, least preferred to most preferred, ...) for a number of conditions.


Data Field Descriptions

Question Text/Heading: In the block that has the default text “Question Text / Heading”, you can type any statement that can describe a situation or setting for the questions that follows.

Row Questions: Each line contains the question or statement text for which a response would be selected. If you would like to add another line item immediately following an existing line, merely click on the “+” box.

Column Headings: Each line contains the heading that represents a portion of a range that can be selected for each row question. If you would like to add another line item immediately following an existing line, merely click on the “+” box.

Limit to One Response per Column: Selecting this option only permits a single selection for a given column across all the row questions. This option may be used independently or in combination with the Limit to One Response per Row option.
